Medical Eligibility
To be medically eligible for sports in the 2023-2024 school year, student athletes must have a NMAA sports physical dated on or after April 1, 2023 on file with the athletic trainer. All physicals for the 2023-2024 school year must be submitted directly to the athletic trainer. Incomplete physicals will not be accepted.
Going To The Doctor
Anyone seeing an outside healthcare provider must bring a note back to the athletic trainer. All notes must include a diagnosis and what the student athlete can/ cannot do. Please use the forms below to ensure all information is included in your note.
